What does GP-led women’s health cover?

A general practice approach to women’s health works as the first port of call. The GP coordinates screening, imaging, lab tests and lifestyle review under one roof, then refers to a specialist only when the case needs it.

How a GP-led pathway works at Mediway

Most women’s health concerns start with symptoms or a routine screening interval. A 20-30 minute GP consultation reviews history, conducts the relevant examination or sample collection, orders bloods or imaging where indicated, and explains results at a follow-up.

For care that exceeds GP scope, such as IUD insertion, IVF, complex menopause management or biopsy, we provide a referral letter and coordinate booking with a partnered specialist or imaging centre. Reports return to the GP for plain-language interpretation and onward planning.

Cervical screening: Pap smear & HPV testing

Two complementary tests for cervical cancer prevention. The Pap smear examines cervical cells for abnormality; the HPV test detects high-risk HPV strains in cervical cells. Singapore’s ScreenForLife programme recommends Pap from age 25, with HPV testing from age 30.

Pelvic imaging: transabdominal & transvaginal ultrasound

Pelvic ultrasound assesses the uterus, ovaries and pelvic structures. Two views are available: transabdominal (over the lower abdomen with a full bladder, used as the screening view) and transvaginal (a small probe placed in the vagina with an empty bladder, used for detailed assessment of the uterus and ovaries).

Bone health: DEXA scan

Dual-energy X-ray absorptiometry (DEXA) measures bone mineral density at the hip and lumbar spine. Postmenopausal women experience accelerated bone loss; routine screening from age 65 is standard, with earlier scans for women with risk factors.

Earlier scan if

Risk factors warrant earlier DEXA: menopause before age 45, fragility fracture, family history of hip fracture, BMI under 19, long-term oral steroid use, hyperthyroidism or chronic kidney disease. The GP will recommend timing at consultation.

Hormone & fertility testing

A blood panel covering FSH, LH, AMH (ovarian reserve), oestradiol, prolactin and TSH gives a structured view of reproductive and hormonal function. Use cases: irregular cycles, fertility planning, perimenopause assessment, and PCOS workup.

Contraception consultation

A confidential GP consultation covering oral and short-acting contraception, plus a referral pathway for long-acting reversible methods. The visit reviews medical history, suitability for each method, and side-effect counselling.

GP Scope

Available at GP level: combined oral contraceptive pill, progestogen-only pill, contraceptive patch, contraceptive injection, emergency contraception (Levonorgestrel up to 72 hours, ulipristal up to 120 hours), and period delay (norethisterone) for travel or events. Suitability consultation, missed-pill guidance and side-effect review included.

Specialist Referral

IUD (copper or hormonal) and contraceptive implant insertion are performed by a specialist O&G. We provide the referral letter and coordinate booking with a partnered specialist if you choose a device-based method.

Menstrual health: irregular periods, heavy bleeding, PCOS

Most cycle disturbances respond to a structured GP workup before specialist referral. Cycle norms run 21-35 days with a 2-7 day bleed; deviations beyond that range, with pain or anaemia, warrant assessment.

First-Line GP Workup

A 30-minute consultation with cycle history, pelvic examination where appropriate, and bloods covering FSH, LH, TSH, prolactin and full blood count. Pelvic ultrasound is added when symptoms suggest fibroids, endometrial pathology, or polycystic ovary morphology. Findings are reviewed at follow-up with a clear management plan or referral.

Red Flags

Book promptly if you experience: bleeding through a sanitary product every hour for several hours, large clots, anaemia symptoms (breathlessness, dizziness, pallor), bleeding between periods or after intercourse, or any post-menopausal bleeding. PCOS is diagnosed using the Rotterdam 2003 criteria (any two of: oligo or anovulation, hyperandrogenism, polycystic ovary morphology).

Menopause assessment

Perimenopause typically begins in the early 40s and lasts 4-8 years before the final menstrual period. A GP-led assessment reviews symptoms, considers hormone testing where helpful, and guides lifestyle and treatment options aligned with NICE NG23.

GP-Led Assessment

The visit covers vasomotor symptoms (hot flushes, night sweats), sleep and mood changes, cycle irregularity, urogenital symptoms, bone health and cardiovascular risk. FSH and oestradiol testing is added selectively, since hormone levels fluctuate widely in perimenopause and are not always diagnostic. First-line guidance focuses on lifestyle, bone protection (calcium, vitamin D, weight-bearing exercise) and DEXA where indicated.

HRT Pathway

Hormone replacement therapy (HRT) initiation is referred to a partnered specialist for review of risks, benefits and dosing. Once stabilised on HRT, ongoing prescriptions and reviews can be managed at Mediway. Non-HRT options (lifestyle, SSRIs for hot flushes, vaginal oestrogen for urogenital symptoms) can be initiated at the GP visit.

01 How much does a women’s health screening cost at Mediway?

02 From what age should I start cervical and breast screening in Singapore?

Singapore’s ScreenForLife programme recommends Pap smear from age 25 (every 3 years if normal), HPV testing from age 30 (every 5 years if normal), and mammography from age 40-49 annually, then every 2 years from age 50. DEXA bone density screening is recommended from age 65. Earlier screening may be advised based on family history, prior abnormal results, or risk factors discussed at the consultation.

03 What is the difference between a Pap smear, HPV test, and co-testing?

A Pap smear examines cervical cells under a microscope for abnormality. An HPV test detects the DNA of high-risk HPV strains in cervical cells. Co-testing runs both on the same sample. From age 30, ScreenForLife recommends HPV testing as the primary screen. Pap smear remains the recommended screen from age 25-29. Both tests are collected the same way: a small brush passed across the cervix during a 5-minute internal examination.

04 Is a female doctor available for women’s health consultations at Mediway?

All our doctors are trained to manage women’s health concerns, including cervical screening, breast examination, contraception, menstrual disorders and menopause. You may indicate a doctor preference at booking via WhatsApp or phone, and we will accommodate where possible based on the schedule. A female chaperone is available for any internal examination if requested.

05 Can I walk in for a Pap smear or do I need an appointment?

Walk-ins are accepted during clinic hours, though waiting times can vary. For Pap smear, mammogram referral, or any package booking, an appointment is recommended so the GP has the full window to take history, perform the procedure and explain next steps. 06 What should I expect at my first women’s health visit?

The first visit is a 30-minute consultation. The GP reviews your medical and menstrual history, current symptoms, contraception, family history of breast or ovarian cancer, and screening history. The relevant examination, sample collection or blood draw is done at the same visit where appropriate. Imaging at partner centres is booked at the end of the consultation. A follow-up consultation 5-10 working days later goes through results and any next steps.

07 How long do test results (Pap, HPV, mammogram, blood work) take?

Indicative turnaround: routine bloods 1-2 working days, hormone panels 2-3 working days, Pap smear cytology 5-7 working days, HPV testing 5-7 working days, mammogram and ultrasound reports 3-7 working days from the partner centre. Urgent results (for example, suspicious mammogram findings) are flagged faster and the GP will contact you directly. Routine results are reviewed at a planned follow-up consultation.

08 Do I need a referral for a mammogram or pelvic ultrasound?

For imaging at partner centres (mammogram, breast ultrasound, pelvic ultrasound, DEXA), a GP referral letter is provided after the consultation. Some imaging centres accept self-referral but most require the letter. The GP’s clinical context on the referral helps the radiologist tailor the report. Mediway coordinates the booking and the report flows back to us for follow-up.
