What is Hepatitis B and why vaccinate?
Hepatitis B is a serious liver infection caused by the Hepatitis B virus (HBV). It spreads through blood, sexual contact, and mother-to-child transmission. Chronic infection can lead to liver cirrhosis, liver failure, or liver cancer over years.
The Hepatitis B vaccine is a recombinant vaccine. It uses a harmless portion of the HBV surface protein to train your immune system. It contains no live virus and cannot cause Hepatitis B infection. Three doses produce long-lasting immunity in ~95% of healthy adults.
Singapore's universal infant Hepatitis B vaccination has been part of the NCIS since 1987. Adults born before 1987 (or anyone unsure of vaccination history) should consider vaccination, especially if they have risk factors (healthcare work, multiple partners, household contact with a carrier, frequent travel to endemic regions).

Hepatitis B vaccination schedule: 3 options
The standard 3-dose schedule works for most people. Accelerated options are available when you need protection faster (e.g. travel, work-pass deadline).

Fastest initial protection (within 3 weeks). 4 doses total. For urgent travel, imminent healthcare exposure, or post-exposure prophylaxis schedules. Requires the 12-month booster for long-term immunity.
Which schedule is right for you? The standard schedule has the strongest long-term immunity and is most commonly used. Accelerated schedules are chosen when timing is critical. Your GP will recommend based on your situation at consultation.

Singapore context: work pass and pre-employment
Some Singapore employers (particularly in healthcare, food services, childcare, and dialysis) require proof of Hepatitis B vaccination or immunity as part of pre-employment medical screening. Work-pass applications may also require Hep B status. If you are applying for a work pass or in a healthcare-adjacent role, consider pre-vaccination testing (HBsAg + anti-HBs) to confirm your current status before starting the vaccination series.
Pre-conception planning: couples planning pregnancy should both be screened for Hep B. If one partner is a carrier, the other should be vaccinated and the infant will need protection at birth.
After vaccination: checking immunity
Most people develop protective immunity after 3 doses. For high-risk individuals, post-vaccination testing confirms immunity.
Anti-HBs antibody testing (post-vaccination)
After completing the 3-dose schedule, an anti-HBs blood test 1-2 months after the last dose checks your immune response:
- Anti-HBs ≥10 mIU/mL: immunity achieved, no booster needed for most people
- Anti-HBs <10 mIU/mL (non-responder): consider a repeat dose or full second series; ~5-10% of adults are non-responders
Who should get post-vaccination testing? Healthcare workers, household contacts of Hep B carriers, dialysis patients, immunocompromised individuals, and infants born to Hep B-positive mothers. Routine post-vaccination testing is not necessary for low-risk adults. Anti-HBs antibody test: from $10 indicative (NETT, GST inclusive at most clinics).
Boosters: For most people with confirmed seroconversion, boosters are not routinely needed. Protection is considered long-lasting (15+ years and likely lifelong). Exceptions include dialysis patients and immunocompromised individuals, who may need periodic anti-HBs monitoring and boosters as indicated.
Twinrix: combined Hepatitis A + B option
Twinrix is a combined vaccine offering protection against both Hepatitis A and Hepatitis B in a single injection.
If you need both Hep A and Hep B vaccination (common for travellers to endemic areas or people at risk from both routes), Twinrix is convenient: same 3-dose schedule, one injection per visit. Per-dose price is slightly higher than Hep B alone, but saves on extra Hep A injections.

Pre-vaccination testing for HBsAg (active infection) and anti-HBs (existing immunity) avoids unnecessary vaccination if you are already immune or already infected. Ask your GP about pre-vaccination screening, particularly important if you had childhood vaccination but uncertain records.
05 Is the Hepatitis B vaccine safe?
The recombinant Hepatitis B vaccine has been used globally for many years and contains no live virus, so it cannot cause Hepatitis B. Side effects are usually mild, such as soreness at the injection site, low-grade fever, or fatigue for 1-2 days. Severe allergic reactions are rare but require urgent medical attention.
06 Should I get Twinrix (combined Hep A + B) instead?
Twinrix is a combined vaccine offering protection against both Hepatitis A and Hepatitis B with the same 3-dose schedule as Hep B alone. It is convenient if you need both vaccines (e.g. travellers to areas with both risks). Per dose is slightly higher priced than Hep B alone. Discuss this during consultation so the doctor can advise which option is suitable for your situation.
07 Do I need a booster after completing the 3-dose schedule?
For most people with confirmed seroconversion (anti-HBs ≥10 mIU/mL), boosters are not routinely needed; protection is long-lasting. Exceptions include dialysis patients, immunocompromised individuals, and some healthcare workers with ongoing exposure. Post-vaccination testing 1-2 months after dose 3 confirms immunity.
08 What if I miss a dose?
Don't restart, the schedule is flexible. A delayed dose can be given as soon as possible, then continue the remaining doses at appropriate intervals. The minimum intervals are: 4 weeks between dose 1 and 2; 8 weeks between dose 2 and 3; 16 weeks between dose 1 and 3. Your GP will adjust your schedule.
09 Can pregnant women get the Hepatitis B vaccine?
Hepatitis B vaccine is not live and may be given during pregnancy when clinically appropriate, especially if there is exposure risk. Discuss with your GP if you are pregnant, planning pregnancy, or unsure of your vaccination status.
