What is a DEXA scan?

DEXA (Dual-Energy X-ray Absorptiometry) uses two low-dose X-ray beams to measure bone mineral density. It is the gold standard test recommended by the World Health Organization and the International Society for Clinical Densitometry (ISCD) for diagnosing osteoporosis and assessing fracture risk.

The scan measures bone density at the lumbar spine and hip: the two sites most prone to osteoporotic fracture. Some scans also include the forearm. Results produce a T-score (comparison to a young healthy adult) and a Z-score (comparison to age-matched peers) that inform diagnosis and treatment decisions.

DEXA is quick (10-20 minutes), painless, and uses extremely low radiation, about 0.001 mSv, roughly a few hours of natural background radiation and significantly less than a standard chest X-ray. No injections, no enclosed scanner, no sedation.

Who should have a DEXA scan?

Singapore MOH and international guidelines recommend DEXA screening for adults at risk of osteoporosis. Mediway GP will assess whether a scan is appropriate for your situation.

Standard screening recommendations:

  • Women aged 65+: all women should have a baseline DEXA
  • Men aged 70+: all men should have a baseline DEXA
  • Post-menopausal women under 65 with additional risk factors
  • Men aged 50-69 with risk factors
  • Any adult who has had a low-impact fracture

Risk factors that warrant earlier DEXA:

Post-menopausal women: oestrogen decline accelerates bone loss
Family history of osteoporosis or hip fracture in parent
Previous low-impact fracture (fall from standing height or less)
Long-term oral steroid use (3+ months of prednisolone or equivalent)
Low body weight (BMI under 19) or significant weight loss
Current smoker or heavy alcohol use (3+ units/day)
Early menopause (before age 45) or surgical menopause
Chronic conditions: rheumatoid arthritis, coeliac, hyperthyroidism, hyperparathyroidism
Certain medications: aromatase inhibitors, androgen-deprivation therapy, long-term PPI
Post-cancer treatment that affected bone health

Understanding your DEXA results

DEXA reports a T-score comparing your bone density to a healthy young adult. The World Health Organization (WHO) classification:

Normal

T-score −1.0 or higher

Bone density within the expected range. General bone-health advice: adequate calcium and vitamin D, weight-bearing exercise, and avoiding smoking and excess alcohol.

Osteopenia

T-score between −1.0 and −2.5

Reduced bone density: not yet osteoporosis, but at elevated fracture risk. Lifestyle interventions, address underlying risk factors, repeat DEXA in 2-5 years. Medication considered if additional risk factors.

Osteoporosis

T-score −2.5 or lower

Significant bone density loss. Fracture risk is high. Treatment typically includes calcium and vitamin D, plus pharmacological therapy (bisphosphonates, denosumab, or others). Repeat DEXA in 1-2 years to monitor response.

Results also include a Z-score comparing your bone density to age-matched peers, useful for assessing whether bone loss is due to age or another cause. Your GP will interpret the full report and recommend appropriate next steps.
