What is vaccine immunity testing?

A blood-based laboratory test that measures specific antibodies against vaccine-preventable diseases, to confirm whether you are immune from prior vaccination or natural infection.

Definition

A vaccine immunity test (also called a titre check or serology titre) measures the level of IgG antibodies in your blood. IgG develops 2 to 8 weeks after vaccination or natural infection and persists for years, indicating long-term protection. Each pathogen has a published cut-off value above which the laboratory reports you as immune.

Five tests run from a single venepuncture at Mediway: MMR (measles, mumps and rubella IgG), Hepatitis B surface antibody (anti-HBs, quantitative), varicella zoster IgG, Hepatitis A IgG (qualitative), and dengue IgG. The dengue test serves a dual role: confirming past dengue exposure and screening for Dengvaxia eligibility.

Samples are processed on HSA-registered immunoassay platforms (CMIA or ELISA) at an accredited Singapore reference laboratory. Standard turnaround is 1 to 3 working days, with a signed digital report sent by email.

Every immunity test reports a titre against a published cut-off. Where your number sits on the axis below decides what happens next.

Your antibody level is too low to confirm protection.

Past vaccination may have waned, the response may have been incomplete, or you may not have completed the full schedule. Either way, your body cannot reliably defend against the pathogen yet.

Schedule a catch-up vaccination. Repeat serology 4 to 8 weeks after the final dose to confirm response.

Right at the cut-off

Your titre is in the borderline zone. Result is indeterminate.

e.g. anti-HBs 9 mIU/mL

Borderline can mean immunity is rising, fading, or sitting at the assay’s grey zone. A repeat in 4 weeks usually settles it. Some patients also benefit from a single booster.

When to test

Timing matters: test too early after a vaccine and you risk a false negative. Below are the windows we recommend in clinic.

Pre-vaccination

Anytime is fine

If you simply want to confirm whether you are already immune from childhood vaccines or past infection, draw the blood whenever it suits you. No timing constraint.

Post-Hep B

1 to 2 months after final dose

For Hep B, post-vaccination anti-HBs is typically taken 1 to 2 months after completing the 3-dose series, per CDC guidance. Earlier testing risks a false negative.

Post-MMR or post-varicella

4 to 6 weeks after second dose

For MMR and varicella, allow at least 4 to 6 weeks after the second dose for IgG to develop fully. Hep A immunity check follows the same 4 to 6 week window after the booster dose.

01 Employer use Do I need an immunity test before getting a Work Permit or S Pass?

For Work Permit and S Pass medicals, the standard panel is HIV, syphilis and a chest X-ray for tuberculosis (per MOM). Vaccine immunity titres are not part of the routine work pass medical, but specific employers (hospitals, childcare, food handling) may request additional measles, varicella or Hepatitis B immunity. For migrant domestic workers, MOM’s June 2025 ruling requires evidence of measles immunity (vaccination record OR positive measles IgG OR completed MMR vaccination at a Singapore GP clinic). 02 Result What if my anti-HBs is below 10 mIU/mL, do I need a Hep B booster?

An anti-HBs titre below 10 mIU/mL means you are below the protective threshold. The standard approach is to give one Hep B booster dose and repeat anti-HBs 1 to 2 months later. If the titre is still below 10 mIU/mL, we typically complete a full 3-dose Hep B series. A small fraction of adults are non-responders and may need an alternative schedule. Our doctor will advise based on your titre, prior vaccination history and any underlying medical conditions.

03 Timing How soon after my MMR or Hep B vaccine should I check my titre?

For Hep B, anti-HBs is typically checked 1 to 2 months after the third (final) dose of the standard 3-dose series, per CDC guidance. For MMR and varicella, immunity checks are usually performed at least 4 to 6 weeks after the second dose. Hep A immunity follows a similar 4 to 6 week window after the booster. Earlier testing risks a false negative because IgG has not yet developed fully.

04 Pregnancy I’m pregnant and my GP wants a rubella or varicella IgG, can I do that here?

Yes. Rubella IgG (within our MMR panel) and varicella IgG are both available as part of antenatal serology screening. We do not vaccinate with live vaccines (MMR, varicella) during pregnancy. If you are not immune, vaccination is scheduled post-partum, typically before discharge from hospital. See our premarital screening page for the combined panel.

05 Vaccine Who can receive Dengvaxia in Singapore?

Dengvaxia (CYD-TDV) is HSA-registered for individuals aged 12 to 45 years with documented prior dengue infection. Vaccination of dengue-naive individuals is contraindicated because of the risk of antibody-dependent enhancement on subsequent natural infection. A dengue IgG serology test is therefore the standard pre-vaccination screen. Eligibility decisions involve other clinical factors as well, please speak with our doctor for individual assessment.

06 HCW I work in healthcare, what immunity proof does my employer require?

Most Singapore healthcare employers require documented immunity to measles, mumps, rubella, varicella and Hepatitis B. The MMR full panel ($120), varicella IgG and Hep B anti-HBs together cover this requirement. Some employers also request HBsAg (to screen for active Hepatitis B carriage) alongside anti-HBs. Tdap status is occasionally requested but proof of recent vaccination (within 10 years) is usually accepted in lieu of a titre. Please check your specific employer’s pre-employment health checklist.

07 Records I lost my childhood vaccination record (NIR or HealthHub), can blood tests replace it?

Yes. Singaporeans can first try to retrieve their childhood vaccination record from HealthHub (powered by HPB’s National Immunisation Registry). If your records are incomplete or pre-date NIR digitisation, an immunity titre check is the recognised alternative. A positive IgG result is generally accepted by employers and schools as evidence of immunity, with the same legal standing as a vaccination certificate. We provide a signed digital report for your HR or school records.

08 Process Can I do all 5 immunity tests in a single blood draw?

Yes. All 5 immunity tests on this panel (MMR, Hep B anti-HBs, varicella IgG, Hep A IgG, dengue IgG) are processed from one SST gold tube, approximately 5 ml. One venepuncture covers the entire panel. The lab uses different aliquots from the same serum sample for each assay, so there is no need for multiple needle insertions.

09 Process How long do the results take and will I get a signed report?

Standard turnaround for all 5 tests is 1 to 3 working days. You will receive a signed digital report by email, reviewed by Dr Qiao Yufei. The report lists each antibody titre against its published cut-off, with a clear immune / equivocal / susceptible interpretation. The signed PDF format is acceptable for school, HR, MOM submission and most travel requirements.

10 Insurance Are immunity tests claimable under MediSave or insurance?

MediSave does not routinely cover immunity titre testing done at the patient’s request (for personal travel, employment or curiosity). Coverage may apply when serology is ordered as part of a broader medical workup for a specific clinical indication. Most private insurance and corporate health plans cover immunity testing when documented as medically necessary or when bundled with annual health screening. Check your policy or speak with our admin team for help with claim documentation.
