What is HIV?

HIV (Human Immunodeficiency Virus) is a virus that attacks the body's CD4 immune cells, the cells that help the body fight infection. Without treatment, HIV progressively weakens the immune system and can lead to AIDS (Acquired Immunodeficiency Syndrome).

The good news: HIV is no longer a death sentence. With modern antiretroviral therapy (ART), most people diagnosed with HIV today can expect a near-normal lifespan and, when the virus is fully suppressed, cannot pass it to sexual partners, known as "U=U" (Undetectable = Untransmittable).

In Singapore, HIV is a manageable chronic condition with well-established clinical pathways, government subsidy schemes, and confidential care options at both public and private clinics.

How HIV spreads (and how it doesn't)

HIV is transmitted through four main routes in Singapore.

Sexual contact
Unprotected vaginal, anal, or oral sex with an HIV-positive partner whose virus is not fully suppressed.
Shared needles
Injecting drug use, unsterile tattoo or piercing needles, or needle-stick injuries in healthcare.
Mother-to-child
Pregnancy, childbirth, or breastfeeding. Antenatal screening + ART reduce risk to under 1%.
Blood transfusion
Extremely rare in Singapore: all donated blood is screened.

HIV does NOT spread through:

  • Hugging, shaking hands, sharing food or utensils
  • Sharing toilets, swimming pools, or gym equipment
  • Mosquito or insect bites
  • Kissing (saliva does not transmit HIV)
  • Sneezing or coughing
  • Casual social contact of any kind

HIV symptoms by stage

HIV progresses through three clinical stages if untreated. Many people have no symptoms for years, which is why testing is the only way to know your status.

Stage 1 · 2–4 weeks post-exposure

Acute HIV infection

  • Fever, night sweats
  • Sore throat, swollen lymph nodes
  • Rash, muscle aches, fatigue
  • Mouth ulcers

Many people have no symptoms or mistake them for a cold. HIV is highly transmissible at this stage.

Stage 2 · years to decades

Chronic (asymptomatic) HIV

  • Often no noticeable symptoms
  • Virus continues to replicate
  • Immune system slowly damaged
  • Can last 10+ years untreated

Regular testing is the only way to detect HIV in this stage.

Stage 3 · without treatment

AIDS

  • CD4 count drops below 200
  • Opportunistic infections (PCP, TB, Kaposi's sarcoma)
  • Severe immune compromise
  • Life-threatening complications

With modern ART started early, progression to AIDS is preventable.

Consistent condom use during vaginal, anal, and oral sex remains the foundational prevention method. Condoms also reduce risk of other STDs including chlamydia, gonorrhoea, syphilis, and hepatitis B.

Medication access and subsidies

Antiretroviral medications are available through public schemes (MediShield Life, MOH subsidies, Medical Assistance Fund for eligible citizens). Most patients can access affordable ART; our team helps you navigate schemes during treatment consultations.

Partner disclosure

Singapore's Infectious Diseases Act requires disclosure to sexual partners in certain contexts. Our team provides guidance on how to have these conversations safely, plus partner-notification information sheets.

Insurance and employment

Travel

Singapore no longer restricts travel for HIV-positive people (rules changed in 2015). Check destination country rules if travelling abroad.

Note on positive results: Singapore law (Infectious Diseases Act) requires doctors to notify MOH of confirmed HIV-positive diagnoses for public-health surveillance. This notification happens after a confirmed positive diagnosis and is separate from testing: if you are only screening and results are negative, no notification occurs. Frequently asked questions

Can HIV be cured?
Not currently. HIV can be fully suppressed to undetectable levels with lifelong antiretroviral therapy (ART), but there is no cure. Research into functional cures continues globally.
How long after exposure can I test for HIV?
The earliest reliable window depends on the test. HIV RNA/DNA PCR detects from ~10–14 days. 4th-generation antigen/antibody rapid tests detect from ~3 weeks. 3rd-gen rapid and antibody lab tests detect from ~3–4 weeks. A conclusive negative is typically confirmed at 3 months. See our testing page for test-specific windows.
What's the difference between PrEP and PEP?
PrEP (Pre-Exposure) is taken regularly BEFORE potential exposure (daily or on-demand) to prevent HIV. PEP (Post-Exposure) is a 28-day emergency course taken AFTER a potential exposure (within 72 hours) to prevent infection.
Is PrEP safe long-term?
Yes. PrEP (tenofovir-based medications) has been used globally since 2012 with strong safety data. Rare side effects include kidney function changes and bone density shifts, monitored with quarterly blood tests.
Will Mediway notify MOH, my insurer, or employer if I test positive for HIV?
Mediway does not notify insurers or employers without your permission. For confirmed HIV-positive diagnoses, Singapore's Infectious Diseases Act requires doctors to notify MOH for surveillance purposes. Negative tests are never reported.
How much does HIV treatment cost long-term?
Modern ART regimens cost approximately $150–$600+ per month without subsidies. Singapore citizens and PRs may qualify for public-sector subsidies significantly reducing costs. Our team helps navigate these at your treatment consultation.
Can I still get life or health insurance if I'm HIV-positive?
Existing policies generally continue. New policies may face exclusions or higher premiums. Always check policy terms; do not disclose status on applications without understanding the consequences.
Is U=U real? Can someone with suppressed HIV not transmit the virus?
Yes. "Undetectable = Untransmittable" (U=U) is well-established science. When a person with HIV has an undetectable viral load on ART, they cannot transmit HIV to sexual partners. This requires consistent treatment and regular viral-load monitoring.
If my partner has HIV, should I take PrEP?
Usually yes, if your partner is not on consistent ART or does not have confirmed undetectable viral load. If your partner is undetectable (U=U), transmission risk is effectively zero, but PrEP adds additional safety. Discuss with our doctor at a consultation.
What do I do if I think I was just exposed to HIV?
